The Ashfield Police, Ashfield Board of Health along with the Council on Aging are hosting the Second Annual Ashfield Flu Clinic on Wednesday, October 7, 2020 from 1 PM to 4 PM.  It will be held inside the bays at the Ashfield Fire Department located at 420 Main Street.    

  • It is open for all residents 9 years of age or older.  
  • It is available by appointment only.
  • Masks/face covering, and social distancing is required.  

If interested call 413-628-4441 Ext: 1 to leave a message with your name, address and contact number for a call back to schedule an appointment.

This service will be billed to your insurance provider so please download the required “Community Off-Site Vaccine Administration Record (VAR)” form and have it completed before arriving to your appointment.
